Progress in ultrasound-guided surgery may improve breast cancer treatment

ScienceDaily (Oct. 26, 2012) ? When surgeons operate to remove a tumor, determining exactly where to cut can be tricky. Ideally, the entire tumor should be removed while leaving a continuous layer of healthy tissue, but current techniques for locating the tumors during surgery are imprecise. Now a multidisciplinary team from the University of California, San Diego, is developing an alternate means of precisely tagging breast cancer tumors for removal or targeted destruction.

They will present the results of their investigations at the AVS 59th International Symposium and Exhibition, held Oct. 28 -- Nov. 2 in Tampa, Fla.

Breast cancer is the most common female cancer in the U.S., and the main cause of death in women ages 40-59, according to UptoDate, an information service for clinical physicians. Over a lifetime, 1 in 8 women in the U.S. is expected to develop breast cancer. Despite great strides in survival, there is trauma associated not only with the disease, but also with its treatment. Many women want to avoid a full mastectomy, but conventional breast-conserving approaches, such as lumpectomy, can be arduous. Up to 25 percent of lumpectomies require a second surgery to excise the entire tumor.

The UCSD team is working on a better method for tagging tumors that should reduce the need for follow-up surgeries. The researchers developed iron-doped -- and therefore biodegradable -- silica micro/nano spheres for implanting into the body as ultrasound contrast markers to guide a surgeon using ultrasound during breast lumpectomy. Additionally, the particles can also be used to destroy tumor tissue with high intensity focused ultrasound (HIFU) ablative therapy, an approach used elsewhere in the world to treat prostate cancer and used in the U.S. to treat uterine fibroids.

If breast tumors are precisely marked, the number of second surgeries can be decreased by 50 percent, according to published studies using radiative tumor markers. Because the gas-filled nanoparticles that the researchers developed make tumors easier to see, they hold the potential for increasing surgical precision with a safe agent. Once injected into the breast cancer tumor, they stick, rendering the tumor more visible with contrast-enhanced ultrasound.

"We are trying to improve the markers surgeons use so they can pull the tumors out with more precision and ease, while reducing trauma for the patient," explains Alex Liberman, the PhD graduate student in the materials science and engineering program who has taken the concept from test tube to animal models. Adds his advisor, chemical physicist Andrew Kummel, PhD: "We are using these particles for two applications. In the short term we are injecting them into breast tumors to enable surgeons to halve the number of second surgeries by readily locating the tumors in the operating room with low- power ultrasound imaging. In the long term, we want to inject the particles intravenously, have them stick to the tumors, and then ablate the tumors by blowing up the particles with high intensity focus ultrasound which is called HIFU."

As now performed, the lumpectomy requires a surgeon to extract tumors through incisions in the breast with the aid of guide wires that protrude out of the breast to help locate the tumor. The wires are prone to movement, and therefore yield imprecise results. Furthermore, the wires are inserted while the patient is awake, which is unpleasant for the patient.

The next step for the team involves conducting more animal tumor studies. Those results will determine if the particles are suitable technology to submit to human clinical breast cancer trials as a localizing agent to guide lumpectomy surgery or even for HIFU therapy.

"Traffic light" codes on food packaging will soon denote whether food is good for us (green), just OK (amber), or perilous (red), making the return to the nursery almost complete.

Products will be judged on fat, salt, sugar and calorie content ? no mention of carbohydrates, oddly.

Unveiling the plan, Broxtowe MP and Health Minister Anna Soubry said the system will help us make healthier food choices.

Oh really? How long did it take them to cook up this idea then? I don't think it will make people think twice about the food they regularly put in their shopping basket.

Especially those who have neither time, money nor inclination to stock up on fresh, (and expensive) wholesome ingredients.

The forthcoming traffic light plan is bound to stall as its ratings are categorised "per serving". Er ? OK then. Hands up those who always eat a carefully weighed and measured amount of food? The system is doomed as we'll be comparing apples with oranges.

That Mediterranean diet full of delicious olive oil that supposedly helps us all live longer? It's bad! Synthetic, low-calorie cup-a-soup or fizzy "diet" drinks? Good! Confused? You will be.

Miss Soubry's also stirred things up further by saying food portions in shops and restaurants are too large, singling out cakes and croissants in particular. So forget "supersize", go "Soubry-size" instead.

I think they've got it wrong. We need tastiness ratings instead. Let me suggest low-fat spread gets a "sad face", butter a "smiley" one. Or how about a beige colour rating for bland food such as crispbread?

Why stop there? Perhaps the think tank gurus can also apply a health rating to those other forbidden pleasures: rubbish telly. A little warning that your brain might turn to mush watching X-Factor, TOWIE or any Peter Andre show could be a genuine public service. It still wouldn't stop millions tuning in though (I'm one of them), just like the traffic lights won't stop the majority buying pizza, sausage rolls or chocolate biscuits.

Food-wise, chefs tell us to keep it simple; MPs are taking this to new levels and it's hard to stomach being treated like a simpleton.

When it comes to food intake, surely the best thing of all is variety ? eating all the colours of nature's bounty to get the full complement of nutrients. I don't think we need to be spoon-fed the idea.

I wonder if those fine examples of health and fitness in Westminster will be abiding by the new guidelines at all? I'll be keeping a close eye on the contents of my own MP's trolley when I spot him at the local supermarket.

Oh, some news just in: I can now reveal that you can't in fact nail jelly to the wall (low-calorie: amber light) and that one-legged ducks (very fatty: big red light) actually do swim round in circles. Don't scoff now!

adaptive skiingLike most of us, I didn't fully realize the extent of the daily hassles and challenges faced by those who use a wheelchair, prosthetic, or other mobility aid until it became somewhat personal. I'm fortunate to have two people in my life who've been an enormous source of both education and inspiration, and I'm writing this piece because of them. A little bit of background is in order:

When I moved to Vail in 1995 to attend culinary school, I became friends with Darol Kubacz, a young Forest Service employee. Darol had broken his back in a motorcycle accident about 18 months prior; at the time of his injury, he was in the Army, working in Special Ops. He was already an experienced outdoorsman who enjoyed scuba diving, climbing, and hiking. Despite the physical challenges and fairly recent onset of his paralysis, he made a huge impression on me with his positive, non-defeatist attitude.

Darol's job with the Forest Service entailed trail assessment for the handicapped, while in his personal life he'd already undertaken a number of adaptive sports, including the aforementioned activities he'd enjoyed prior to his injury. He'd also started alpine skiing (he broke his neck in a skiing accident in 2000, but fortunately sustained no additional physical or neurological damage).

Darol became my workout buddy, and he was the first friend I'd ever had who was in a chair. Through him, I learned a lot about what it means to live with a limitation. Mainly, he impressed upon me that, to a certain extent, it's possible for humans to overcome physical limitations. I'm surprised he doesn't have, "Where there's a will, there's a way," carved into his chest.

arm bikeToday, Darol works as a part-time adaptive hiking guide in Phoenix (he and his clients use off-road arm bikes),and is working on launching an adaptive paragliding program. He's climbed Mt. Kilimanjaro ?twice, summiting once? entirely under his own power, to raise awareness for his foundation, Freedom for Life. Following his ski accident, he has, he says, "Learned to embrace a more intimate experience with nature, that's less about speed and adrenalin, and more about being in the moment." Hence his passion for off-road bikes.

I met my friend Tony 12 years, ago, when I was living in Berkeley and working as a farmers market vendor. A loyal customer, Tony is also a documentary filmmaker and graphic designer. He's quadriplegic, the result of a teenage diving accident. Tony has partial use of his arms, and until his accident, was a competitive surfer. Until a few years ago, however, he'd never been able to get back on a board due to some medical issues he was dealing with.

Today, at a freakishly youthful 48, Tony is an avid surfer and skier (that's him at Alpine Meadows, in the photo at the beginning of this story), thanks to several amazing adaptive sport programs. He says he's in the best shape of his life, and his jones for salt water and snow matches that of any able-bodied enthusiast.

Living in the outdoor adventure mecca of Boulder as I do, I'm also in an epicenter of outdoor adaptive recreation programs. With my locale and both of these inspiring and incredible guys in mind, I wanted to provide a round-up of top adaptive sport centers across the country.
